(Plus include your title, author name, contact information, pitch + page numbers in the manuscript.)
If the title has changed, explain in the cover letter.

Yes, agents sometimes send me redlined drafts and non-final versions, titled āUSE THIS ONEā or numbered gibberish. Itās not going to make or break a submission, and agents can help, but itās good to build strong habits for naming files for your own peace of mind!#QueryTip When sending a full manuscript per an agent's request, name your file:
